That is the rule here, and it is not optional. The Advertising Standards Authority's codes say every party to an ad is responsible for it being identifiable, and the Fair Trading Act makes a post that hides its commercial nature a misleading representation.

Yes, and that is the point. Vouch pays for recommendations from actual customers, so the receipt in your own name is what separates this from being sent a free box. If you did not pay for it, it is not a Vouch post.
There is no minimum. The bottom band is nought to 250 followers and it has a published rate like every other band. Nine followers is fine.
Yes, every time, and it has to be upfront and readable rather than buried in hashtags. Ad, advert or advertisement all work. It is one of the six checks, so a post without it does not get paid.
We check your follower count against your account when we verify your first post. If the band is different, we pay the band you are actually in and we tell you before anything else happens.
Into the bank account in your own name, once your post passes the six checks.
Vouch pays the published rate, with no joining fee and no cut taken from it. How that income is taxed depends on your own situation, so check with an accountant.
